The Court has reviewed the expedited petition filed July 9, 2024 by Petitioner Roshawnda Dawson (Petitioner) on behalf of Claimant Rhe’yanna Zeigler, age 14. The Court denies the petition without prejudice for the following reasons:

 

Item 13a(4) does not reflect the total amount of the medical expenses sought to be reimbursed.

 

Attachment 13a does not identify the other medical lien holders that make up $2,800, or explain any reductions/payments.

 

Petitioner requests $2,250 in attorney fees which represents 25% of the gross settlement. The Court finds this amount to be reasonable.

 

The net balance in item 16 must be corrected.

 

In MC-351, Petitioner must mark 8b1 and write the net settlement amount. In MC-355, Petitioner must mark 5b.

 

 

Moving party is ordered to give notice.
