PARTY’S REQUESTS

DISCUSSION
Section 12 is incorrectly filled out. In Section 12(a)(1), Petitioner states that total medical expenses before reductions equal $525.46. This directly conflicts with the information provided in Section 12(5)(b)(I), which states that Kaiser Permanente Alone charged $9,663.36. The Court denies the application.
