Petitioner has failed to file a birth certificate confirming the identify of Petitioner, age of Petitioner, and the identity of Petitioner’s parents.  As noted in the January 3, 2022 and February 22, 2022 hearings, failure to file a birth certificate would result in the petition being denied and dismissed. 

The petition is dismissed without prejudice.  Petitioner may file a new petition and file a birth certificate or criminal history assessment.  Court Clerk to give notice.
