The petition is denied on the ground that Petitioner has failed to complete all necessary forms, including NC-100 (petitioner did not print name under section 7(f).) The petition is dismissed without prejudice.  Petitioner may file a new petition and properly completed NC-100.

In addition, the Court previously notified Petitioner that there is an indication from Probation’s criminal history assessment that Petitioner has an active outstanding warrant: PETITIONER HAS AN OUTSTANDING BENCH WARRANT; $NO BAIL (SAN BERNARDIND) CS #FVI20000274. Petitioner was required to clear the outstanding bench warrant and file evidence of such before the Court will rule on the petition for name change.  As Petitioner has failed to do so the instant petition is DENIED
