Petitioners have failed to complete all necessary forms, including one NC-110 for each person whose name is proposed to be changed.  There is no NC-110 attached to the petition at all for Nicole Clark or Paloma Jaymes Clark.  Petitioners did not fill out item 7(f) of the NC-110 for Oliver Bennett Clark.

The petition and OSC are continued to February 27, 2023 at 9 am.  No later than one week prior to the continued hearing, Petitioners must file properly completed NC-110s for all three – Nicole Clark, Oliver Bennett Clark, and Paloma Jaymes Clark.   Failure to do so will result in denial of the petition without prejudice.
