Petitioner has failed to provide any form of identification tied to a fingerprint on file, which is necessary for Probation/the Court to run a criminal history assessment.  Specifically, Petitioner failed to identify the state that issued the driver’s license or ID.

The petition is continued to August 12, 2024 at 9:00 am for Petitioner to file a name change criminal history assessment form with a California Driver’s License, California ID, or submit a license or ID from another state and specify said state.

Petitioner must file a new criminal history assessment form (LASC CIV 226) fixing this error no later than three weeks before the continued hearing date so that the Clerk’s Office can establish Petitioner’s identity.
