Petitioner is over 13 years old and has failed to submit a criminal history assessment form.  The petition is CONTINUED to September 9, 2024 at 9:30 am to provide Petitioner with an opportunity to submit a criminal history assessment form so that The Court Clerk may verify Petitioner’s criminal history, including whether Petitioner is a registered sex offender.  Within one week, Petitioner must submit a criminal history assessment form.  The Name Change Clerk is ordered to run and complete a criminal history assessment prior to the continued hearing.  Court Clerk to give notice.

There is an additional defect that must be corrected.  No proof of publication has been filed.   The petition is CONTINUED to September 9, 2024 at 9:30 am for the additional reason that Petitioner must file proof of publication.  Petitioner must file proof of publication no later than one week before the continued hearing date.  Failure to do so will result in denial and dismissal of the petition without prejudice.
