It appears that Petitioner has failed to submit a criminal history assessment form.  The petition is CONTINUED to November 4, 2024 to provide Petitioner with an opportunity to submit a criminal history assessment form so that The Court Clerk may verify Petitioner’s criminal history, including whether Petitioner is a registered sex offender.  Within one week, Petitioner must submit a criminal history assessment form.  The Name Change Clerk is ordered to run and complete a criminal history assessment prior to the continued hearing.  Court Clerk to give notice.
