Discussion
The proposed settlement and requested attorney’s fees are reasonable.
Here, Petitioner proposes to disburse the requested fees and costs to Counsel and medical costs to Claimant’s insurance provider. Petitioner proposes to disburse the remaining $120,808 to a blocked account and properly filed a form MC-355.
Petitioner must attach medical records and identify the insurance provider being reimbursed. Additionally, Item number 17(2) in the MC-350 is incomplete. If Petitioner did have an agreement with Counsel, Petitioner must attach a copy of the agreement.
