DECISION
The hearing is continued to permit Petitioner time to cure the deficiencies noted below.
Corrected documents must be made at least five court days in advance of the continued hearing date.
The parties are ordered to appear on the date of the hearing to set a continued hearing date.

Discussion
Although Petitioner filed a proposed order, it was not made on a form MC-351. Petitioner must submit form MC-351.
With respect to form MC-350, the following items need to be corrected.
Item number 3: Box b must be checked because Petitioner is the guardian ad litem
Item number 11(b): Name of Defendant and amount to be paid in the chart is missing.
Item number 12: box 4 must be checked because Petitioner will receive money under the proposed settlement.
Item number 13: States medical expenses to be paid from proceeds is 0. However, there is a lien for [Redacted] to be paid to Claimant’s physical therapist. Petitioner must fill out Item number 13 to reflect the lien.
