DECISION
The Court finds that the settlement and the requested attorney’s fees are reasonable. However, the matter is continued so that the issues discussed below may be corrected.
The parties are to appear on the hearing date to discuss a continued hearing date with the Judicial Assistant.
Corrected papers must be filed at least five court days in advance of the hearing date.

Discussion
Total medical expenses were $30,703. Claimant’s private insurance paid for $20,509 and the carrier has not requested reimbursement. Form MC-330 item #13(a)(2) states $1,125 remains to be paid in medical expenses from the proceeds of the settlement. Documents provided in support do not specify who will be paid. There must be a specification of who will be paid.
Costs are not specified, just totaled. Form MC-330 item number #15 states these expenses are shown in item #14b, but #14b is blank.
Counsel provides a fee declaration in support of the fee. Cal Rules of Court, Rule 7.955(c). However, he did not attach a copy of the retainer agreement as required by MC-330 item #18(a).
Counsel is also ordered to provide the (proposed) Order Approving Compromise (Form MC-351) completely filled-out, as Counsel has failed to do so.
