Box 3b: must be checked since Petitioner is the claimant’s Guardian Ad Litem.
Petitioner intends to use $621.82 to pay remaining medical expenses. Petitioner also indicates that Claimant’s medical expenses were paid by Medi-Cal. The medical reports attached show that Claimant’s medical expenses were much more than just $621.82. It is unclear from Petitioner’s responses on form MC-350 whether the other medical expenses were fully paid by Medi-Cal and which medical provider remains to be paid from the proceeds of the settlement. MC-351 Item number 7(c), Petitioner lists the medical provider to be paid simply as “Medical.” This needs to be clarified.
