DECISION
The petition is denied without prejudice. The Court notes that the claimant turns 18 in May 2023.

Discussion
On December 6, 2022, the Court continued this matter to allow Petitioner to correct deficiencies with the petition. However, there a still deficiencies with the petition.
Petitioner filed an outdated form MC-350 from 2011. Petitioner must use updated forms.
Petitioner submitted a Medi-Cal lien letter. However, Petitioner again failed to submit Claimant’s medical records pertaining to his injuries from the accident.
Petitioner again filed an MC-355, but the form is unnecessary because Petitioner intends to use the remaining settlement funds to purchase an annuity.
MC-350 Item number 9c(2) should not be checked because the full amount of net settlement will be used to purchase an annuity.
The terms of the annuity were attached to the petition, though mislabeled because Petitioner submitted an outdated form MC-350.
