The Default Judgment Application is denied without prejudice because (1) a proposed judgment on Form JUD-100 was not provided; (2) section 2 of Form CIV-100 was not filled out; and (3) the declarations provided by the Plaintiffs do not explain how their damages were calculated (this is only explained in their memorandum).
