On August 22, 2024, the Court reviewed petitioner’s initial petition and outlined a list of deficiencies for the petitioner to correct. On October 25, the Court reviewed the new petition and noted that many deficiencies listed in the August 22, 2024, order remained uncorrected. On November 12, 2024, a new petition was filed; however, some deficiencies still remain.  

Item 10 of the petition remains incomplete, as the name of the defendant is missing under item 10(b) and needs to be provided. Item 10(c) still needs to be completed. Since item 11(b) is checked, stating that an individual other than the claimant is receiving a settlement, then 11(b)(5) needs to be completed regarding the amount of settlement that other plaintiffs are receiving. Items 17(c)-17(f) need to be completed. Further, attachment 11(b)(6) still needs to be provided. 

Item 4 of the proposed order needs to be completed.
