The numbers in item12a needs to be amended as it currently doesn’t add up.
The total medical expenses to be reimbursed from the proceeds do not appear to include the figure listed under 12b.
The record retrieval cost of $9,566.95 seems unusually high; counsel is instructed to provide an explanation for this expense.
Petitioner is hereby ordered to correct the deficiencies noted above no less than 5 days prior to the new court date set below. Failure to do so may result in sanctions.¿¿
