TENTATIVE 

The court examined the minor’s compromise package and notes the following deficiencies:

Petition

·        Attachments 8, 11b(3), and 17a need to be provided.

·        Item 10 contains the wrong amount, listing the total settlement amount instead of the amount for the claimant. This needs to be revised.

·        Item 11b(1) needs to be revised as it includes the total settlement amount to all parties.

·        Item 12a(3-5) are missing and need to be completed.

·        Item 12b(5)(b) does not add up and needs to be revised.

·        Item 13b is empty and the claimant does not request costs, but under 16d, the claimant requests $250 in costs. This inconsistency needs to be corrected throughout the petition.

Proposed Orders

·        Item 8 of the proposed order does not add up. Claimant is not requesting cost under item 8a(2), but the total amount under 8b appears to include costs.

·        Item 4 of the proposed order to deposit into a blocked account needs to be revised to reflect the correct amount.

Petitioner is hereby ordered to correct the deficiencies noted above no less than 5 days prior to the new court date set below. Failure to do so may result in sanctions.¿
