The Court reviews the petition and finds the settlement and the attorney's fees reasonable. However, the proposed order contains the following minor deficiencies: Reimbursement for medical expenses, including the name of each payee, address, and amount, should be provided in Attachment 8a(3); costs should be listed under Item 8a(2).

The Court CONDITIONALLY GRANTS the petition upon filing of an amended proposed order addressing the deficiencies listed above no less than 5 days prior to the new court date set below. Failure to do so may result in sanctions.
