Ruling:  Continued NLT 60 days to allow Plaintiff to correct deficiencies.  Moving party to give notice.

Rationale: 

The Statement of Damages must be filed using form CIV-050, not pleading paper.  Default is void unless the proper forms are used.   

Plaintiff failed to address the deficiency regarding the request tor $45,000 damages which exceeds the amount in the statement of damages and does not put Defendant on sufficient notice of its liability.  Plaintiff may not seek damages that exceed the amount in her statement of damages.

All other requests are allowable and reasonable.
