The motion to be relieved as counsel for Plaintiff, Edna Fuentes, is DENIED without prejudice. Counsel for Plaintiff’s counsel has submitted all of the mandatory judicial council forms (i.e., MC-051, MC-052, and MC-053). However, item 3b of the declaration has not been filled out. Further, not all hearing dates are listed on the declaration and proposed order. (CRC Rule 3.1362(e).) It appears there are outdated hearing dates listed. Moreover, the declaration states that there is a trial set for March, while the proposed order states trial is not yet set.
