RELIEF REQUESTED: An order relieving attorney Samer Habbas as counsel for Plaintiff Jahn Forte in this action. 

RULING:   

On 11/19/24, attorney Samer Habbas filed the instant motion seeking an order relieving attorney Habbas as counsel for Plaintiff Jahn Forte in this action due to an irreparable breakdown in the attorney-client relationship.  The amended declaration filed in support of the motion indicates that Plaintiff’s address was confirmed within 30 days of the filing of the motion and proofs of service have been filed indicating that Plaintiff has been personally served and served by mail with the motion papers and the other parties have been served by mail with the motion. 

However, the proposed order does not include a telephone number for Plaintiff as required in No.6.  Upon attorney Habbas providing/including a telephone number for Plaintiff in the proposed order, the motion will be granted.
