The Court has reviewed the petitions by Petitioner Jayson Alan Tengonciang on behalf of Claimants Avery Tengonciang and Zoey Tengonciang, and continues the hearing to June 13, 2023 at 1:30 PM in Department 32 to permit Petitioner time to address the following issues:

 

·         Sections 12a and 12b:  Petitioner has not advanced evidence of the total medical expenses of Claimants, nor evidence of the purported negotiated reductions.

·         Petitioner has failed to file a MC-351 form for each Claimant

·         Petitioner has failed to file a MC-355 form for each Claimant

 

Petitioner shall file and serve amended petitions and file forms MC-351 and MC-355 for each Claimant on or before June 6, 2023 .  Petitioner shall provide notice of the Court’s orders and file a proof of service regarding the same.
