ORDER 
Petitioner, 
The proposed decree requests
change of name and order recognizing gender change and issuance of a new birth
certificate. While this is the correct form for the decree Petitioner is
required to use Judicial Council Forms NC-200 (Petition) and an Order to Show
Cause on form NC-125. Petitioner is also required to submit LASC CIV Form 226 to
complete a Name Change Criminal History Assessment. The NC-200 petition is
exempt from the requirements for publication. Code Civ. Proc., § 1277.5 subd. (b). A
hearing shall not be set unless an objection is timely filed and shows good
cause for opposing the name change. Code Civ. Proc., § 1277.5 subd. (c). 
Petitioner is ordered to complete
the forms referenced above and file them with the court. Upon receipt, the
court will issue the Order to Show Cause (Form NC-125). Absent any objections within
six weeks of making the order, the court will enter the order granting the
petition without a hearing.  Code Civ. Proc., § 1277.5 subd.
