1.    Doe dismissal does not use the proper form (CIV-110).

2.      2.     JUD-100 requests $150,000 in damages, however the CIV-100 requests $130,467 in damages.

3.    The medical bills attached as exhibits to the summary of the case includes repeat charges, and charges for issues not related to this case, making it extremely difficult to itemize the charges to determine if they match the amounts stated in the summary.
