Plaintiff’s counsel filed all the required forms. However, the declaration and proposed order have the incorrect hearing date of November 15, 2022 instead of December 15, 2022. Also, proof of service shows that only the notice of motion and motion to be relieved as counsel and declaration in support were served on Defendants’ counsel. Plaintiff’s counsel must serve all the requisite forms on the client and Defendants. (CRC 3.1362(d) [“The notice of motion and motion, the declaration, and the proposed order must be served on the client and on all other parties who have appeared in the case.”].)

Conclusion 

              Motion is DENIED without prejudice.
