Tentative: The Court has reviewed the request for default judgment, and is CONTINUING the matter to allow counsel to resubmit a default packet to correct the following issues:

(1)  Plaintiff must file a new request for court judgment on form CIV-100 and only check the box for “Court Judgment,” not both “Court Judgment” and “Entry of Judgment.”

(2)  Damages have not been proved. There is no evidence to support the amount of damages sought. For example, there are no medical bills, or proof that any future medical expenses are required, such as a doctor’s note. There is also no evidence to support the loss of earnings claim, such as a note from Plaintiff’s supervisor, indicating how much she makes and how much time she has missed from work.

(3)  Statement of Damages served on Defendant must be filed with the Court.
