The declaration of Brent S. Silverman fails to state (1) that the applicant is a licensee in good standing in those courts; and (2) that the applicant is not currently suspended or disbarred in any court. (Cal. R. Ct., 9.40(d)(3)-(4).) 

Additionally, no proposed order has been filed.
