Moving Counsel failed to file and serve a [Proposed] Order Granting Attorney's Motion to Be Relieved as Counsel-Civil using mandatory form MC-053 as required by California Rules of Court, rule 3.1362(e). 

Moving Counsel is order to file a proposed order forthwith, and serve it on Defendant Belvin Pastor and all other parties who have appeared in the case in accordance with California Rules of Court, rule 3.1362, subdivision (d).
