RULING: Denied.

Counsel for plaintiff Greg Salazar moves to be relieved as counsel of record, due to “irreconcilable differences.”

The “notice of motion and motion” was not filed on form MC-051. (Cal. Rules Court, rule 3.1362(a).) Use of the form is mandatory. The motion is therefore denied.
