 The Motion to be Relieved as Counsel of Record, filed by Mary Jean Sumell, Esq. of Jay S. Rothman and Associates, as to the representation of Plaintiff Samuel Ornelas, primarily complies with C.R.C. 3.1362.  But the Proposed Order submitted with the Motion needs to be corrected, to show the client’s last known address and phone number, to list all pending motions and the corresponding hearing dates, and the MSC and trial dates, and to check Box 5(a).

Moving counsel is to promptly submit a revised Proposed Order; the Motion will be granted upon approval thereof, with relief to be effective upon filing of a proof of service of that signed order on the client.
