The Motion to be Relieved as Counsel of Record, filed by moving counsel Laurel N. Haag of Buchsbaum & Haag, LLP as to the representation of Plaintiff Shawn Woods, primarily complies with the requirements of C.R.C. 3.1362. However, the proposed Order submitted as ROA 32 fails to provide the required information in ¶¶ 3(b), 5(a), and 6-9. The Motion will therefore be GRANTED, upon submission of an amended proposed Order which provides the required information, with relief to be effective upon the filing of a proof of service reflecting service of the signed Order on the client and on defense counsel.
